Accommodations at UMFK

UMFK provides accommodations for students with documented disabilities. If you need accommodations, you must apply for them through the Disability Services and Accommodations Office located in the Student Success Center. Accommodations will vary depending on your needs as they fall under Federal law, the Rehabilitation Act of 1993, the Americans with Disabilities Act of 1990, and the Maine Human Rights Act established the rights of individuals with disabilities.

Steps to Apply

  1. Complete the Student Accommodations Request form
  2. Submit supporting documentation of disability. We encourage the use of the Medical Documentation form. Information needed must include:
    1. Diagnosis with date
    2. Signature of medical provider
    3. Recommendations
  3. Student Rights and Responsibility form
  4. Meet with the Disability Services and Accommodations Coordinator

Please Note:

  1. All documentation must be complete and submitted prior to approval.
  2. Accommodations may take up to 14 days for processing and approval.
  3. Emotional Support Animal (ESA) documentation must be completed in full prior to the proposed arrival to campus.
  4. Accommodations can never fundamentally alter essential course requirements as determined by the course instructor.
    1. Essential requirements are the outcomes (including skills, knowledge, and attitudes) all students must demonstrate with or without using accommodations.

Student Accommodations Request Form

You must request accommodations before the beginning of each semester – accommodations are not automatically renewed. Specific accommodations are determined based on the nature of the disability and the requirements of each course.
