This form is only for distance education students taking courses at the University of Maine at Fort Kent and who do not attend any classes on campus.

If you are a student from away taking UMFK classes, you may fill out the form below. With the information you provide us we will determine if you need a UMFK Card/BengalBadge and if so, provide you one in the mail. Please note that you can only receive a card if you have your immunization records and other forms up to date. These forms need to be handled by the campus at which you are taking your primary classes.

However, you do not have to have the BengalBadge to access online resources such as databases and ebooks. You can access these resources through the UMFK portal or choosing the portal access when prompted. However, having a BengalBadge allows you to request books (see Borrowing Privilages) and provides a back up to access electronic resources if the Portal in not available.

Know that once you apply it can take up to a week to get your card activated and mailed.
