This form is only for distance education students taking courses at the University of Maine at Fort Kent and who do not attend any classes on campus.
If you are a student from away taking UMFK classes, you may fill out the form below. With the information you provide us we will determine if you need a UMFK Card and if so, provide you one in the mail. Please note that you can only receive a card if you have your immunization records and other forms up to date. These forms need to be handled by the campus at which you are taking your primary classes.
Getting a card means you will have access to the UMFK databases. These databases allow you to search through thousands of resources to retrieve articles and other materials available either in full text online, or in abstract and citation form so you may request to borrow the item.
If you are enrolled primarily at another campus and taking one or two online classes at UMFK, we will forward your information to Off-Campus Services who will issue you a "multiuser" card. The multiuser card will grant you access to FK databases for the semester you signed up for classes at Fort Kent.
Don't wait to apply. Once you apply it can take up to a week to get your card activated and mailed.
