Residence Hall Questionnaire & Dining Hall Contract

This form is ONLY required if you intend to live on campus and is required before we can make a room assignment.

Immunization & Medical Forms

All students are required to show proof of immunization and verify insurance information. We also ask that you provide basic medical information for our Health Clinic to have on hand in the case of an emergency on campus.

* Nursing students must go to Verified Credentials to set up their online immunization records. For all other majors, please complete the General Immunization Form.

Students may submit the general immunization form to the Student Affairs office by fax or email.

Student Health Insurance

A new change in the health insurance verification requirement for students enrolled at University of Maine System (UMS) schools will take effect on August 1, 2021. The UMS student health insurance page outlines your options for accessing health insurance coverage and information about free wellbeing resources if you are not already covered under another plan. Please note: some University programs, such as allied health, aviation, and intercollegiate athletics, require health insurance for program participation. These changes do not apply to Graduate Assistants and International students. For additional information, please contact the Business Office at (207) 834-8650 or umfkbus@maine.edu.

New Student Intake Form

This form will help our Student Support Services Office to provide you with the best academic support possible.

Activate Your Email

If you have not activated your email account, please do so. All official UMFK notices will go out using this “@maine.edu” email account. If you need assistance activating your account, contact our Help Desk at (207) 834-7818 or email help@maine.edu.

Attend Orientation

Orientation is a great way to meet other new students and learn about our campus. Sessions are held throughout the summer for the Fall semester and in January for the Spring semester. Our Student Affairs staff will send more information before a scheduled Orientation session. If you would like additional information about Orientation, contact Student Affairs at elysha.berube@maine.edu.

Financial Aid

If you have not applied for Financial Aid, you can apply online. The priority filing date for completing a Financial Aid file to UMFK is March 31. We recommend submitting the Free Application for Federal Student Aid (FAFSA) to the federal processing center by February 1. Although we give consideration for assistance at any time during the year, it is in your best interest to apply as early as possible after October 1. For additional information, contact our Financial Aid Office at (207) 834-7605 or umfkaid@maine.edu.

Financial Affidavit of Support Form (INTERNATIONAL STUDENTS ONLY)

All International students are required to show proof of sufficient funds before UMFK can issue an I-20. The Financial Affidavit of Support provides specific instructions on how to complete the form.
